Wellness Center

A completed medical history and physical examination are required prior to arrival on campus.

Please do not assume records will be forwarded from previous schools, physicians, or ROTC departments. They will not.

Valley Forge Military College cannot accept physical examinations dated prior to May 31st of the student’s incoming academic year.

Students may submit any of the following as acceptable documentation:

• A current sports physical completed within the required timeframe

• A current Full DoDMERB physical that falls within the required examination dates

• A current Army “Safe to Train” document that falls within the required examination dates

Students who are unable to provide qualifying DoDMERB or “Safe to Train” documentation are are required to obtain a standard sports physical prior to arrival on campus.

All required medical documentation must be submitted before students may fully participate in cadet training, physical activities, or athletics.

All immunization records must be submitted prior to arrival on campus.

• Physician offices

• Schools

• State immunization systems

The following immunizations are required before arrival.

1. Tetanus-Diphtheria-Pertussis / Tdap – Students must provide proof of protection against tetanus, diphtheria, and pertussis, also known as whooping cough.
2. Polio – Students must provide documentation showing they have completed the required polio immunization series. After age 18, polio is not required unless the student has not had previous polio vaccinations.
3. Hepatitis B – Students must provide proof of Hepatitis B immunization. Three vaccinations are required, or students may provide a blood test showing immunity.
4. Measles, Mumps, and Rubella, also known as MMR – Students must provide proof of MMR immunization. This vaccine helps protect against measles, mumps, and rubella.
5. Varicella, also known as Chickenpox – Students must provide proof of varicella vaccination. Two vaccinations are required, or students may provide a documented date of disease or a titer showing immunity.
6. Meningococcal, also known as Meningitis – Students must provide proof of meningococcal vaccination covering serogroups A, C, Y, and W-135. This vaccine helps protect against meningococcal disease, which can spread more easily in residential campus settings.
7. Tuberculosis Screening / TB/PPD or QuantiFERON Gold – Students must provide documentation of tuberculosis screening. TB/PPD results must include the date placed, date read, and result in millimeters. If a QuantiFERON Gold test was completed, the result must be attached. If the screening is not completed before arrival, TB/PPD testing may be completed on campus for a $25 fee added to the student’s account.

ALL CADETS MUST BE COVERED BY ACTIVE MEDICAL INSURANCE.

Students who do not provide proof of insurance are required to enroll in an insurance plan. International students are required to carry approved coverage. The Wellness Center can assist with insurance options. 

In the event of:

• Emergency treatment

• Hospitalization

• Specialist referral

• Prescription needs

A copy of the student’s insurance card (front and back) must be submitted with Health Center paperwork.

Please Note:

All co-pays, deductibles, prescription costs, and uncovered expenses remain the responsibility of the student and/or parent/guardian.

Cadets must complete the following Health Forms by August 1st.

1. Physical Form
2. Immunization Record
3. Heath Insurance 
4. Consent for Medical Treatment

If a cadet must return home for medical reasons, a Medical Leave request should be initiated as soon as possible through the appropriate school official.

Prior to returning to campus, students may be required to provide documentation from the treating medical provider clearing them for return.

Failure to provide required documentation may result in the absence being considered unexcused.

Cadets are responsible for managing and securing their own approved medications while residing on campus.

Students requiring accommodations or assistance related to ongoing medical conditions or medication management should contact the Wellness Center prior to arrival.

Illegal substances and unauthorized controlled substances are strictly prohibited on campus.
